Requests for Client Information
All requests for client information are reviewed and managed by the Risk and Quality Team at Bendigo Community Health Services (BCHS).
The Risk and Quality Team will oversee and coordinate our response to any requests for client information, in line with our legislative and privacy obligations.
Clients Requesting Access to Own Health Records
BCHS clients must complete this form if they would like to request to access their own health records held by BCHS. We will respond to these requests under the requirements of the Health Records Act 2001 (Vic).
Request to Release Client Health Information to a Third Party
BCHS clients must complete this form if they would like any of their health information released from us to another third party (including another health or other service provider, insurer etc). We will respond to these requests under the requirements of the Health Records Act 2001 (Vic).
Request to Transfer Client Medical Records to BCHS
BCHS clients must complete this form if they would like their medical records transferred to us from another medical practice. We will respond to these requests under the requirements of the Health Records Act 2001 (Vic).

Information Sharing Scheme Requests
The Family Violence Information Sharing Scheme (FVISS) and Child Wellbeing Information Sharing Scheme (CISS) enable the sharing of information between authorised organisations to assess and manage family violence risk.
The purpose of these schemes is to keep victim survivors safe and hold perpetrators to account. The schemes authorise the sharing of information to:
- Assess risk (family violence assessment purpose)
- Manage the risk, including through ongoing risk management (family violence protection purpose)
- Promote the health and safety of a child or group of children.
Information Sharing Entities (ISE) and Risk Assessment Entities (RSE) are encouraged to submit requests for information under the Child Information Sharing Scheme (CISS) and Family Violence Information Sharing Scheme (FVISS) through BCHS Information Sharing Scheme INCOMING REQUEST Form.
